This site hosts a short questionnaire for people who may have an injury claim in Utah. It is operated as an advertising and intake service. It is not a law firm, and using it does not create an attorney-client relationship.

Your answers are used to determine whether your situation matches the types of cases our partner law firm accepts. If it does, your contact details and answers are sent to that firm so an intake specialist can contact you about a free case review. If it does not, your information is not forwarded.
By submitting the form you consent to being contacted by phone, text, or email about your inquiry. You can opt out at any time by replying STOP to a text, or by emailing us at the address below.
We share submitted information with the partner law firm handling intake for your case type, and with service providers that operate this site and our customer records system on our behalf. We do not sell your information to unrelated third parties.
We use advertising tools, including the Meta pixel, to measure the performance of our ads. These tools may set cookies and report that a form submission occurred. They do not receive the medical or incident details of your answers.
We keep submissions for as long as needed to operate the service and meet record-keeping obligations. You may request a copy of your information, or ask us to delete it, by emailing contact@slightwrks.com. We will respond within a reasonable time.
This site is not directed to anyone under 18, and we do not knowingly collect information from children.
